Definition
The frequency and quality of meaningful human review of AI-generated outputs before they become consequential decisions or actions. High HVD means humans genuinely inspect AI work. Low HVD means AI outputs pass through unchecked.
Why It Matters
AI systems produce plausible-looking outputs that can be wrong, biased, or hallucinated. Human Verification Density is the primary safeguard. When HVD collapses, execution becomes unverifiable — the organisation is running on AI outputs nobody reviewed. This is the first and most direct path to Execution Debt accumulation.
What teams do about this
When Human Verification Density is low, teams show Courage dropping (nobody speaks up when AI outputs look wrong), Impression Management rising (people say what leadership wants to hear rather than flagging what AI actually produced), and Openness to flagging mistakes declining.
